ЭФФЕКТИВНОСТЬ ЧРЕСКОЖНОГО ВМЕШАТЕЛЬСТВА И МЕДИКАМЕНТОЗНОЙ ТЕРАПИИ У ПОЖИЛЫХ БОЛЬНЫХ С МНОГОСОСУДИСТЫМ ПОРАЖЕНИЕМ КОРОНАРНОГО РУСЛА

Aim: to study the characteristics and 12-month outcomes of various treatment strategies in multi-vessel lesion of coronary arteries in a comparative perspective. Methods: we examined 102 patients with multivessel coronary artery disease. All patients were divided into 2 groups: stenting of symptom-related coronary artery, and if necessary, other coronary arteries (from 1 to 4 stents), and received only optimal medical therapy (OMT). During the year both groups were under observation. In particular, fixed endpoint such as death, recurrent myocardial infarction (non-fatal), the progression of coronary artery disease, development and progression of heart failure (HF), re-hospitalization, the need for surgical intervention (CABG). Results: For the major end points, in respect of them between groups, no significant differences were observed. Of deaths (from cardiovascular and other causes) in both groups was not observed, the same applies to reinfarction, stroke and bleeding. Re-hospitalization due to progression of coronary and heart failure was noted in 10.8 % of cases in the group of stenting patients and 11.4 % of cases in group OMT. Coronary artery bypass grafting was only performed in 2 patients from the group of stenting patients i in group OMT, the need for surgery arose. Conclusion: PCI in elderly patients with CHD is a highly effective and safe method of therapy. The use of PCI helps to reduce the frequency of hospitalizations and improve the clinical course of angina pectoris. While maintaining the proper degree of adherence to therapy, optimal medical therapy based on modern clinical recommendations OMT based on the results of a one-year follow-up is comparable to that of PCI by major endpoints.
