ИНГИБИТОРЫ АНГИОТЕНЗИНОВЫХ РЕЦЕПТОРОВ И НЕПРИЛИЗИНА В ПРАКТИКЕ ЛЕЧЕНИЯ СЕРДЕЧНОЙ НЕДОСТАТОЧНОСТИ

The aim. Evaluation of the clinical and hemodynamic effect of the angiotensin receptor inhibitor and neprilysin in patients with CHF of non-coronary etiology at the stage of inpatient and outpatient treatment. Materials and methods. The study included 64 patients with non-coronary etiology CHF III-IV NYHA classes, hospitalized for the development of decompensated HF, which were divided into 2 groups: Group I comprised 32 who were prescribed sacubitril/valsartan in an average dose of 28,45 ± 15,5 mg / day. Patients of the II group (n = 32) took ACE inhibitors in the recommended doses. Results. There was an improvement in the functional status of patients in the form of a reduction in overall weight, a significant reduction in heart rate, with stabilization of blood pressure, improvement in 6MWT data (from 84.00 ± 36.30 to 280.00 ± 83.21 m, in 2 groups – from 82.00 ± 46.32.32 to 240.00 ± 76.35), a decrease in the total of Mareev score (from 8.67 ± 3.58 to 3 ± 1.54 and 8.62 ± 4.58 to 4.67 ± 3.27 points in both groups, respectively), improvements in the quality of life for MQoL (from 86.25 ± 20.12 to 18.20 ± 12.14 in group I and from 88.25 ± 26.78 to 28.12 ± 15.13 in Group II), as well as a positive impact on echocardiography indicators. Conclusion. The use of standard HF therapy in the inpatient and outpatient stages is accompanied by an improvement in the indicators of quality of life and clinical and functional status, especially noticeable when ARNI is prescribed.
