ОСОБЕННОСТИ ИНТЕНСИВНОЙ ТЕРАПИИ ПОЛИОРГАННОЙ НЕДОСТАТОЧНОСТИ ПОСЛЕ КАРДИОХИРУРГИЧЕСКОГО ВМЕШАТЕЛЬСТВА

The article is presented a clinical case of successful intensive therapy of patient A 52 years old after prosthetics of the mitral valve, plasty of the fibrotic ring tricuspid valve, aortocoronary shunting in conditions of artificial circulation and cardioplegia. Complicated intraoperatively acute left ventricular failure and hypocoagulation syndrome, which was a detonator of the development of an early syndrome of multiple organ failure (cerebral, respiratory, hepatic-renal). The patient was in prolonged artificial ventilation of the lungs. Later was developed purulent-inflammatory complication in connection with the admission of hospital infection (against the background of immunodeficiency), contributing to the development of secondary complications from the lungs-the fan-associated tracheobronchitis. Entire all of respiratory system (use of a premium respirator, non-invasive artificial ventilation) for intensive therapy (correction of water-electrolyte balance, substitution, inotropic, neurotropic therapy, correction of cardio-renal syndrome) and targeted antibiotic therapy, taking into account the resistance of the local flora. That complex made to allow to achieve a satisfactory clinical course of multiple organ failure and the outcome of the critical state.
