MODERN APPROACHES TO DIAGNOSIS AND TREATMENT OF ACUTE INFLAMMATORY INTESTINAL POSSIBILITY.

Timely diagnosis and selection of the optimal surgical treatment option for patients with acute intestinal obstruction are one of the most difficult and urgent problems in emergency abdominal surgery [8,12]. Back in 1971, Yu.M. Dederer wrote that “Acute intestinal obstruction has earned the sad glory of a very heavy in course, difficult to diagnose and unfavorable for the outcome of the disease” [2]. The formation of adhesions of the abdominal cavity is one of the main causes of intestinal obstruction, isolated in a separate form - acute adhesive intestinal obstruction [16]. This is due to a number of inflammatory processes, after injuries and surgical interventions. The problem is becoming increasingly urgent due to the increasing number and volume of various surgical interventions [8]. Peritoneal adhesions are increasingly common in young patients, leading to the development of a number of complications. Abdominal adhesions are one of the most common causes of intestinal obstruction with a high level of mortality without a tendency to decrease [9]. The incidence of adhesions varies from 67% to 93% after surgical abdominal operations and is almost 97% after open gynecological interventions. In modern abdominal surgery, the problem of the formation of postoperative abdominal adhesions does not lose its relevance [11,3]. Acute adhesive intestinal obstruction is distinguished by the severity and speed of development of pathophysiological changes in strangulated forms, a variety of clinical manifestations and the associated difficulties of diagnosis, tactical and technical difficulties of surgical treatment and prevention [4,15]. Therefore, postoperative mortality in case of acute adhesive intestinal obstruction is kept at 15%, work capacity is restored only in 40–50% of patients, and after conservative treatment in 30–35% [5].
