ЗНАЧЕНИЕ КЛИНИЧЕСКИХ И ФУНКЦИОНАЛЬНЫХ ПОКАЗАТЕЛЕЙ В ОЦЕНКЕ РИСКА СМЕРТИ У БОЛЬНЫХ ДКМП С ПОСТРОЕНИЕМ ПРОГНОСТИЧЕСКОЙ МОДЕЛИ

Objective: To assess the significance of the clinico-functional indicators for the prognosis of the life of DCM patients and to construct a prognostic model of 1-year mortality. Materials and methods: 274 patients with DCM, aged 16 to 61 years with clinical manifestations of heart failure II-IV functional class (NYHA) of NYHA functional class (FC) by NYHA were examined, the ratio of men and women was 2: 1. Patients under study were observed from 3 to 170 months (compare 36.7 ± 5.6). Endpoints were: death (sudden or progression of heart failure), thromboembolic complications (PE, ischemic stroke, thromboembolism of the vessels of the kidneys and vessels of the lower limbs). Results and conclusions: A sequential evaluation of anemnestic, clinical, instrumental data with the analysis of the life expectancy and the subsequent creation of the predictive model by the multiple stepwise logistic regression method was carried out. It was shown that the family form of this pathology, relatively young age, high NYHA class, low blood pressure, QRS complex change, ventricular arrhythmia of high gradation, low ejection fraction and increase in ESV of LV more than 178 ml were independent risk factors of death in DCM patients.
