ПЕРВЫЕ РЕЗУЛЬТАТЫ ИСПОЛЬЗОВАНИЯ ТОТАЛЬНОЙ АРТЕРИАЛЬНОЙ РЕВАСКУЛЯРИЗАЦИИ НА РАБОТАЮЩЕМ СЕРДЦЕ

Introduction. Suboptimal results demonstrated by venous grafts in the long term period after coronary artery bypass surgery made the use of arterial conduits for myocardial revascularization extremely important. Despite the good results demonstrated by the latter, only 5% of cardiac surgery centers worldwide practice this technique routinely. Material and methods. In the Department of Cardiac Surgery of the RSSPMCK, from October 2017 to March 2018, 15 off-pump CABG procedures with exclusive use of arterial conduits were performed. The mean age of the patients was 60.2 ± 9.3 years (38–77). The majority of patients were male - 13 (86.6%). The risk of surgical intervention, calculated according to the risk-stratification calculator EuroSCORE, was 3.83±4.0% (0.88–21.25%) or 3.43±2.7 (0–10) points. Results. There were no intraoperative conversions or other complications. The average duration of the operation was about 225±55.0 (160–340) minutes. The average number of distal anastomoses was 3.02±0.8 (2–5) per patient. The average ventilation time 5.6 ± 3.1 (2–8 hours). Conclusions. The initial results of the use of total arterial off-pump CABG procedures are satisfactory, although they have been obtained in a small number of patients subjected to careful selection. For further refinement of indications and contraindications to this method, further accumulation of clinical material is necessary.
